Children's Healthcare of Atlanta opened one of the world's largest and most advanced “smart” pediatric hospitals in 2024. A project nearly 10 years in the making. The project required a tight collaboration across the enterprise to ensure the reliability and interoperability of advanced technologies ranging from AI to cutting-edge video. To accomplish this, organizational change and culture needed to be considered given both surface and deeply ingrained technologies.

Session Description:

Healthcare is a significant contributor to pollution and greenhouse gas (GHG) emissions, generates 2 gigatons of carbon dioxide equivalents (CO2e) annually. In the United States, healthcare alone contributes 8.5% of total GHG emissions, and globally, it represents 4.4% of emissions. This exacerbates climate-related health challenges, particularly those affecting marginalized populations. The Joint Commission, the largest regulatory body, announced in 2024 that improving sustainability is one of its strategic goals and launched a sustainability certification. Within healthcare, the operating room stands out as a major source of GHG emissions, accounting for up to 40% of a hospital's total direct emissions. Inhaled anesthetic agents used in surgeries are particularly concerning as they are potent GHGs. These agents have a longer atmospheric lifespan than CO2 (e.g., Nitrous Oxide lasts 104 years in the atmosphere), leading to a significantly high global warming potential. In response to these challenges, Project Spruce (Saving the Planet by Reducing Carbon Emissions), an international consortium of 12 hospitals, came together with the goal of achieving at least a 50% reduction in anesthesia-related greenhouse gas emissions. This initiative employs practice constraints, education, and clinical feedback on protocol improvements by leveraging real-world EMR data via AdaptX. By collaborating towards a common goal, these efforts have successfully removed over 1,900 tons of carbon from the environment. Seattle Children's, the original site for Project Spruce, has dramatically reduced their emissions 10x, equivalent to over 500,000 kg of carbon dioxide annually. These improvements yielded substantial cost savings, exceeding $175,000 per year, without compromising patient outcomes.
