Breaking Down Silos: Enabling Care Delivery at Home across the Care Continuum

Many health systems have multiple, disconnected point solution programs for virtual care delivery, targeting specific conditions and acuity levels in isolation. For example, patients discharged from a Hospital at Home Program, may still have chronic conditions needing management after the acute period through a lower touch program, and vice versa. Or, an oncology patient may have fluctuations in their condition that may warrant different levels of care over the course of their treatment. How do we enable a seamless transition across their journey, and your enterprise while enabling care in the home?
If your organization is already delivering care at home programs, we want to hear how you’re thinking about connecting the dots across programs, considering topics such as: Opportunities to step a patient up and down in acuity across programs, multiple condition management, EHR and reimbursement considerations.
