Data: Sources and Strategies for Achieving Payment & Care Delivery Goals
Driving truly effective care interventions requires a deep understanding of a patient’s journey, including care received at various locations and life experiences outside the four walls of any health system. To achieve this, providers need aggregated data that helps build a more complete picture, both of the individual patient and overall population. With increasing access to a variety of data sources–EHR, claims, HIEs, RPM–providers are now faced with determining how to prioritize and strategize data use based on their organization’s unique care and payment delivery model needs.
