Objective:
Driving truly effective care interventions requires a deep understanding of a patient’s journey, including care received at various locations and life experiences outside the four walls of any health system. To achieve this, providers need aggregated data that helps build a more complete picture, both of the individual patient and overall population. With increasing access to a variety of data sources–EHR, claims, HIEs, RPM–providers are now faced with determining how to prioritize and strategize data use based on their organization’s unique care and payment delivery model needs. At the same time, the pressing need to address health inequity requires the thoughtful collection and analysis of SDoH data to ensure that outreach efforts and interventions are meaningfully tailored to individual and population-level needs.
