Certain features and areas of the CHIME website are available only with registration, login and/or a paid membership. If you are required to register and select a unique login and password (“Personal Login Information”), you must keep your Personal Login Information confidential. Your Personal Login Information is personal to you and you may not allow any third party to use it under any circumstances. CHIME and CHIME Foundation is not liable for any harm caused by or related to the theft, misappropriation, disclosure, or unauthorized use of your Personal Login Information. You must contact CHIME immediately if you become aware of or believe there is or may have been any unauthorized use of your Personal Login Information, or otherwise wish to deactivate your Personal Login Information due to security concerns.

The content of the CHIME website includes: (a) CHIME’s and CHIME Foundation’s trademarks, service marks, logos, brands, and brand names, trade dress and trade names and other distinctive identification (collectively “CHIME Marks”); and (b) information, data, materials, interfaces, computer code, databases, products, services, software applications and tools, text, images, photographs, audio and video material, and artwork; and (c) the design, structure, selection, compilation, assembly, coordination, expression, functionalities, applications, look and feel, and arrangement of any content contained in or available through the CHIME website (the items identified in subsections (i) (ii) and (iii) shall be collectively referred to herein as “CHIME Content”). CHIME Content is the property of the CHIME and/or CHIME Foundation, their licensors, sponsors, partners, advertisers, content providers or other third parties and is protected by law, including United States copyright, trade secret, patent, and trademark law, as well as other state, national and international laws, treaties and regulations. The reproduction, transmission, distribution, sale, publication, broadcast, circulation or dissemination of any CHIME Content by you, or by you through any other person or entity, is prohibited unless express written consent is separately obtained from the CHIME, CHIME Foundation or the owner of such content if the CHIME is not the owner, as applicable. Any use of the CHIME Marks without CHIME’s express written consent is strictly prohibited. You may not alter, delete, obscure or conceal any copyright or other notices appearing in the CHIME Content, including any such notices appearing on any CHIME Content you are permitted to download, transmit, display, print, or reproduce from the CHIME website.

The CHIME website may feature materials, information, products, and services provided by third parties. Any such information, including articles, press clippings, opinions, advice, statements, services, offers or other information made available by third parties such as content providers and other users of the CHIME website are those of the respective third party and not of CHIME, CHIME Foundation or their affiliates. Neither CHIME nor CHIME Foundation makes any representation with respect to, nor do they guarantee or endorse, the quality, non-infringement, accuracy, completeness, timeliness, or reliability of such third party materials, information, services or products.
The CHIME website may contain advertisements of third parties. The inclusion of advertisements on the CHIME website does not imply endorsement of the advertised products or services by CHIME or CHIME Foundation. CHIME and CHIME Foundation shall not be responsible for any loss or damage of any kind incurred as a result of the presence of such advertisements on the CHIME website. Further, CHIME and CHIME Foundation shall not be responsible or liable for the statements or conduct of any third party advertisers appearing on the CHIME website. You shall be solely responsible for any correspondence or transactions you have with any third party advertisers.

The CHIME website may provide links to other sites on the Internet for your convenience. These other sites are maintained by third parties over whom CHIME and CHIME Foundation exercise no control. 8. Information We May Collect and How We May Use or Distribute Your Information
CHIME and CHIME Foundation collects general information about the usage of this website as well as any personal information that you provide. The personal information collected depends on how you choose to make use of our website and our services. We will not distribute this information in a manner inconsistent with this policy unless required by law.
a. General Website Usage Information
To monitor site usage and improve the quality of our website, CHIME and CHIME Foundation gathers general information about the use of the CHIME website. We use web server logs and industry-standard website statistics software to count the number of users who visit the website and to count the number of views of particular pages, as well as to track other general statistics about our website. We track individual users’ visits to the website, however, CHIME and CHIME Foundation do not share this information with third parties. At times CHIME and/or CHIME Foundation may share with third parties the aggregate, anonymous information about overall usage of the website and other Internet services that CHIME or CHIME Foundation collects, in order to provide general information about our Internet services, sell advertising, or establish sponsorships or other business relationships. CHIME and CHIME Foundation do not share any information about specific members.
b. Information Collected to Process Transactions
When visitors to the website or users of our other Internet services apply for or renew membership, register for events, or sign up for services, CHIME and CHIME Foundation will collect personal information necessary to process the transaction. This may include your name, address, telephone and fax numbers, email address, and credit card number or other payment information. The personal information you provide is encrypted and sent to us using a secure method, in order to assure that your personal information is securely and safely transmitted. However, please understand that no one can give an absolute assurance that information intended to be maintained as private, whether transmitted via the Internet or otherwise, cannot be accessed inappropriately or unlawfully by third parties. CHIME and CHIME Foundation have taken and will continue to take reasonable steps to ensure the secure and safe transmission of your personal information.
c. Payment Information
Payment information, such as your credit card details, is collected only to process a transaction and once used, is captured and stored for record keeping purposes in a computer that is not linked to the website. Credit card information is not stored in a manner that would allow you or another online user to reacquire such information. While being used to process the transaction, reasonable measures are taken to ensure your payment information is not vulnerable to theft or interception.
d. Email Addresses and Other Member Information
General. Members are required to provide their email address, contact information and other information required to establish and maintain your membership (“Membership Information”). Membership Information is used by CHIME and CHIME Foundation to support and provide their programs and services to their individual and collective members. In addition, CHIME provides its members and CHIME Foundation and its members with access to Membership Information through CHIME’s directory services. From time-to-time CHIME and CHIME Foundation provide Membership Information to CHIME Education Foundation for the purpose of promoting this affiliate’s programs and services. When contacting CHIME or CHIME Foundation we may ask for certain personal information such as your member ID number, and for members and non-members your name, email address, telephone number, or fax number. When collected for this purpose, the information is used solely to verify identity and to provide the assistance you are requesting.
Use by HIMSS to Promote the CIO Forum. CHIME and CHIME Foundation provide email addresses and other Membership Information to HIMSS to enable HIMSS to market the CIO Forums. HIMSS keeps this information separate from all of HIMSS’ other data and is only used to promote the CIO Forums.
Data Sharing with Third Party Companies. Under some circumstances, CHIME may have an affiliation with third party companies to provide select programs and services to CHIME and CHIME Foundation members. As part of these arrangements, CHIME may share email addresses and other Membership Information with these third party companies to provide the services and programs and otherwise contact our members. If you do not want your Membership Information to be provided to these third party companies, you will need to opt out from sharing this information. Opting out must be made on a total basis, i.e., a member cannot choose to opt out for some but not other third party programs. If you choose to opt out, there may be limitations on the services and programs that will be made available from the third parties with whom CHIME has an affiliation.
Other than the foregoing, CHIME and CHIME Foundation will not distribute or transfer the email addresses or other information of members or non-members to any third party.
e. Members’ Information
CHIME and CHIME Foundation offers members certain features and services through the restricted-access “Members Section” of the website. The Members Section allows members to access and update their Member Profiles, which consists of Membership Information and other personal information we have collected from each member as part of their membership application, and to change their passwords. Member Profiles are maintained for every active member of CHIME and CHIME Foundation in our main membership databases. This information includes information such as member ID number, name, firm/organization, address, telephone and fax numbers, and email address.
The personal information in Member Profiles is occasionally used by CHIME and CHIME Foundation to distribute marketing materials by mail, fax or email. Members can elect not to receive certain printed mailings from CHIME and/or CHIME Foundation, or not to receive any faxes from CHIME and/or CHIME Foundation, or not to receive certain email notices from CHIME and/or CHIME Foundation. However, electing not to receive any of these communications may mean that you will not receive important notices about your membership or products, events, or services that may be useful to you.

f. Information Collected to Secure Access to Members Section
CHIME and CHIME Foundation may also collect a password for each member who uses online services. CHIME and CHIME Foundation may use this information to verify members’ identities in order to prevent unauthorized access to members’ personal information. Only a small number of CHIME and CHIME Foundation personnel and personnel at our third-party technology vendor have access to this information. This information is accessed only when necessary to provide members with online account-related information, provide technical support to members, or diagnose or repair problems with the Members Section software and is not otherwise disclosed to anyone, other than the member or his or her designee, unless compelled by law.
CHIME and CHIME Foundation collect and use the email addresses of members to our listserves for the purposes of sending the email messages that comprise these services. CHIME and CHIME Foundation may occasionally send email messages with surveys regarding these services or promoting other CHIME and CHIME Foundation services to members and non-members. CHIME and CHIME Foundation will not share the compilation comprising the listserve to third parties.
9. Links to Other Web Sites
The CHIME website and many print and electronic publications contain many links to other websites. These other website may send their own cookies to users, collect data or solicit personal information. CHIME and CHIME Foundation are not responsible for the privacy practices or the content of these other websites. Please check with the other sites to determine their privacy policies and how they may collect or use information.
CHIME and CHIME Foundation staff are educated on this policy and obligated to follow it. Access to personal information is limited to select personnel and the information is stored in an encrypted format. We also take steps to obligate third parties to handle information appropriately and confidentially.

CHIME and CHIME Foundation do not offer or sell products or services for purchase by children. If you are under 18 years of age, you may not purchase products or services from, and you should not provide information to, CHIME or CHIME Foundation without the involvement and consent of a parent or guardian.
